Photocopy of your birth certificate

Extracts from your recent birth certificate (2010 or later).

For older extracts, request a new one through the Embassy ($100 USD fee).

Photocopy of your old passport (photo page only)

Registration of fingerprints on the back of the form

All documents must be notarized by an authorized official of a Haitian consulate by appointment only, a notary public, a professional approved by the passport service. i.e. Pastor, Priest, Doctor, Lawyer, Judge… can also sign and seal documents. Send the documents to
